Travel RN Surgery Position - Cleveland, Ohio
Location: Cleveland, Ohio

Facility Type: Level 1 Trauma Center with Vascular, Neuro, and General Surgery units
Contract Length
- 12 weeks
Required Certifications & Licenses
- BLS (Basic Life Support) – American Heart Association only
- ACLS (Advanced Cardiovascular Life Support) – American Heart Association only
- Valid Permanent RN License – Primary source verification required within 30 days of start; temporary licenses not accepted
- License/Certificate Verification – Must be current and verified
Required Health & Immunizations
- COVID-19 Vaccine – Preferred but not required; medical or religious exemptions accepted with approval
- MMR (Measles, Mumps, Rubella) – Positive titers within last 15 years required; booster protocol applies if low titer
- Varicella – Positive titers within last 15 years required; booster protocol applies if low titer
- Hepatitis B – Proof of full series, partial series with declination, or positive titers within 15 years accepted
- Tdap – Vaccination within last 10 years; no declinations accepted
- Influenza Vaccine – Required during flu season (Oct-Mar); declination requires mask-wearing and specific forms
- Fit Test – Completed within 1 year of start; specific approved mask types required
- 2-Step PPD or IGRA (Quantiferon/Tspot) – Negative test within last 6 months; if positive, clear chest x-ray and current TB questionnaire required
- Physical/Health Statement – Pre-employment physical exam within 1 year stating ability to work without restrictions
Background & Verification Requirements
- Criminal Background Check – Within 6 months, including Social Security Trace, County searches, National Criminal Sweep, and Sex Offender Registry
- Ohio Fingerprints – Required for all Ohio residents (5+ years or less than 5 years with FBI check); attestation allowed for pending results
- Education Verification – Highest level verified through third party; international equivalency required if applicable
- Employment Verification – Verification of employment within last 5 years through third party
- OIG Search – Completed within 6 months prior to start
- SAM/EPLS/GSA/OFAC Screening – Completed within 6 months prior to start
- Social Security Trace – If not included in background check, must be completed within 1 year prior to start
- National Database Criminal Background Check – For Ohio residents, FCRA-compliant check required
Testing & Competency
- Drug Screen – 10-panel urine drug screen within 60 days of start; instant drug screens not accepted
- UHS Competency Exam – Must score at least 80%; valid within 1 year prior to start
- UHS Core Competencies – Includes HIPAA, Joint Commission, OSHA topics; 80% passing score required; valid within 1 year prior to start
- Skills Checklist – Clinical skill-specific checklist completed within 1 year prior to start (initial hire only)

- UHS I-9 Attestation – Proof of eligibility to work in the U.S.
- UHS Submission Packet – Resume, 2 professional references (within last 2 years), job application, and UH employment history form
- UHS Badge Photo – Recent headshot with plain background (JPEG format)
- Driver’s License/State ID – Non-expired, required for submission
Summary
This travel RN Surgery position requires weekend night shifts at a Level 1 Trauma center with specialties in vascular, neuro, and general surgery. Candidates must hold current BLS and ACLS certifications, a valid permanent RN license, and meet extensive health, immunization, background, and competency requirements. The contract length is approximately 12 weeks. Compliance with all credentialing and onboarding documentation is mandatory prior to start.
