Job Opportunity: CT Technologist (Night Shift)

Location: Columbus, Ohio
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Work Schedule

  • Night shift: Monday to Wednesday, 11:00 PM – 7:30 AM
  • Every other weekend: 7:00 AM – 7:30 PM

Required Credentials & Compliance for Onboarding

Personal Health History

  • COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption with agency letterhead (declination not accepted for CA or IL). Valid through October 2026.
  • MMR: Proof of 2 vaccinations with dates or primary source IgG titer lab report.
  • Fit Test: Required.
  • Hepatitis B: Must have started vaccination series prior to start date (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Hep B titers not accepted.
  • Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead for flu season 10/1/2024–4/30/2025 and 10/1/2025–4/30/2026 (declination not accepted for CA or IL).
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year (for MI, IN, CT, MA) or within 5 years for other regions.
  • Varicella: Proof of 1 vaccination with date or primary source IgG titer lab report.
  • Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
  • Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days), must include specific substances; no THC allowed.

Professional Requirements

  • Trinity Profile: Resume, skills checklist within a year, and 2 professional references required for submission.
  • Confidentiality & Security Agreement: Signed Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement.
  • Conflict of Interest Disclosure Form: Signed and on file.
  • Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, including SSN verification, dated within 30 days of start.
  • Education Verification: Highest academic education verified by third party screening report.
  • EPLS (Excluded Parties List System) Check: Scanned report dated within past 30 days.
  • GSA/OIG Exclusion Check: Scanned report dated within past 30 days.
  • Integrity and Audit Services Form: Signed shortly before start date.
  • Joint Commission Testing: CORE exam within the last year.
  • NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases and locations.
  • System Access Form: Must be completed and uploaded one week before start.
  • Skills Checklist: Provided by agency for clinical contractors.
  • Specialty Specific Competency Testing: Specialty exam completed within the last year.
  • Code of Conduct Form: Signed and on file.
  • I9 Verification: Standard I9 completed by agency.

Certifications & Licenses

  • BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must include front and back images of card with expiration date.
  • License/Certificate Verification: Must be dated within 30 days of start. Online verification page with machine-generated date required. For RNs and LPNs, state verification via NURSYS preferred. California requires a picture of the hard copy license for RNs.
  • Color Vision Test: Required.

This position requires strict adherence to all credentialing, health, and compliance requirements prior to onboarding.
