Nursing Case Manager - Juneau, Alaska (Contract)
Position Overview
- Specialty: Nursing (Case Manager)
- Location: Juneau, Alaska
- Employment Type: Contract
- Contract Length: 13 weeks
Key Requirements
- Must have an active Alaska RN license at time of application.
- Case management experience within the last 2 years.
- Proof of flu vaccine required.
- Fingerprint clearance required; start date at least 2 weeks after fingerprint receipt.
- Graduate of an accredited nursing school; Bachelor’s degree preferred.
- Basic Life Support (BLS) certification preferred.
- National Case Management certification preferred.
- Minimum 1 year of case management experience.
Job Responsibilities
- Actively manage patient care in collaboration with medical providers and interdisciplinary teams.
- Coordinate multi-disciplinary care conferences for high-risk or complex patients.
- Manage acute, chronic, and health maintenance care needs through EHR review and patient outreach.
- Participate in case management reviews with multi-disciplinary teams.
- Advocate for patients and families to promote decision-making, independence, and self-care.
- Facilitate coordination and tracking of patient referrals within and outside the organization.
- Perform nursing duties including assessment, documentation, education, and medication/therapy administration.
- Maintain annual competencies including safety and infection control.
- Participate in triage and respond to emergency situations as an RN.
- Support specialty clinics by managing referrals, scheduling, and assisting providers during procedures.
- Serve as primary case manager for OB patients, including intake, education, and postpartum/newborn care tracking.
- Coordinate video teleconferencing visits with specialty providers.
- Manage referrals and appointments for Exercise Treadmill Tests.
Skills and Knowledge
- Knowledge of nursing process and ability to apply it in clinical settings.
- Familiarity with tribal health services in Alaska preferred.
- Strong customer service, safety, and infection control principles.
- Proficient in assessment, data collection, and use of medical/computer equipment.
- Effective oral and written communication skills.
- Ability to multi-task, work independently, and problem-solve in a fast-paced environment.
- Physical ability to stand for long periods, bend, stoop, climb, and move quickly.
Additional Information
- Applicants must live at least 50 miles from the facility location.
- Housing and travel expenses are not provided.
- Must maintain compliance with annual competencies and infection control policies.
- Expected to work collaboratively within a multi-disciplinary team and participate in ongoing role development.
This position is ideal for experienced RNs with recent case management experience seeking a contract role in Alaska with a focus on comprehensive patient care coordination and specialty clinic support.
