CT Technologist - Davenport, Iowa

Employment Type: Long-Term Assignment (LTA)
Contract Length: 17 weeks (approximately 4 months)
Shift: Every Other Weekend (EOW)


Job Description

  • 16-week assignment (noting contract length is 17 weeks)
  • Shifts include every other weekend coverage

Required Credentials & Compliance for Onboarding

Certifications

  • BLS (American Heart Association certification required for all direct patient care personnel; must provide front and back card images)
  • ARRT Certification

Licenses

  • License/Certificate Verification dated within 30 days of start (state verification required)

Personal Health History Requirements

  • Fit Test
  • Trinity 10 Panel Drug Screen (9 or 10 panel drug screen completed within 30 days of start; no THC)
  • COVID-19 Vaccine (proof of booster by Sept 2025, declination or exemption accepted except for CA or IL)
  • MMR (proof of 2 vaccinations or IgG titer)
  • Hepatitis B (must have started vaccination series prior to start; titers not accepted)
  • Influenza Vaccine (proof or declination for 10/1/2024-4/30/2025; declination not accepted for CA or IL)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
  • Varicella (proof of 1 vaccination or IgG titer)
  • Physical/Health Statement (exam within last year confirming fitness to work)
  • Tdap (current, non-expired vaccine; declination accepted if no pediatric interaction)

Professional Requirements

  • Trinity Code of Conduct Form (signed)
  • Trinity I9 (completed by agency)
  • Confidentiality & System Security Agreement (signed)
  • Integrity and Audit Services Form (signed shortly before start)
  • Non Employee Upload Form (pages 1-3 and 28 completed)
  • Specialty Specific Competency Testing (copy of specialty exam within last year)
  • PowerPoint Genesis (signature page for Iowa locations)
  • Conflict of Interest Disclosure Form (signed)
  • Criminal Background Check (Federal, Nationwide, County for last 7 years; SSN verification; dated within 30 days)
  • Education Verification (highest academic degree or diploma/GED verified)
  • EPLS (Exclusions report dated within 30 days)
  • GSA/OIG (Exclusions report dated within 30 days)
  • Joint Commission CORE Exam (within last year)
  • NSO (National Sex Offender check dated within 30 days; includes aliases and locations)
  • System Access Form (completed and uploaded one week before start)
  • Skills Checklist (provided by agency for clinical contractors)
  • Child and Adult Dependent Abuse Training (Iowa only)
  • Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)
  • OFAC Sanctions Check (completed within 30 days of start)
