Job Opportunity: RN Cath Lab (LTA)

Location: Mishawaka, Indiana
Contract Length: 13 weeks

Job Description

• Work schedule includes 1 weekend per month and weekday call 4-5 days per pay period (weekend included in total).
• Call coverage: End of shift to 7:30 AM Monday-Friday on scheduled call days.
• Weekend call coverage: End of shift Friday through Monday 7:30 AM.

Required Credentials & Certifications (Must be current and verifiable)

• Licenses:

  • Valid nursing license with verification dated within 30 days of start.
• Certifications:

  • BLS (American Heart Association certification required for all direct patient care personnel; front and back of card required)
  • ACLS (Required for RNs in critical care areas; front and back of card required)
  • NRP (If applicable for Birth Center RNs)
  • NIHSS
  • CPI
• Health & Immunizations:

  • Fit Test
  • Annual TB Questionnaire
  • Trinity 10 Panel Drug Screen (completed within 30 days of start, no THC)
  • COVID-19 Vaccine (proof of booster or approved exemption; declination not accepted in CA or IL)
  • MMR (proof of 2 vaccinations or IgG titer)
  • Hepatitis B (must have started vaccination series prior to start; titers not accepted)
  • Influenza Vaccine (proof or declination; declination not accepted in CA or IL)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (per state requirements)
  • Varicella (proof of vaccination or IgG titer)
  • Physical/Health Statement (exam within last year confirming fitness to work)
  • Tdap (current vaccine; declination accepted if no pediatric interaction)

Professional & Compliance Requirements

• Signed Trinity Code of Conduct Form
• Trinity I9 form completed by agency
• Confidentiality and System Security Agreement signed
• Integrity and Audit Services Form signed shortly before start
• Trinity Joint Commission Testing (CORE Exam within past year)
• Trinity Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification; dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG Exclusion check dated within 30 days
• NSO (National Sex Offender) check dated within 30 days
• System Access Form completed and uploaded one week before start
• Trinity Skills Checklist (provided by agency for clinical contractors)
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days
• Trinity Orientation Guide completed
