Unit Description
- Neuro Critical Care Unit
Required Credentials & Compliance for Submission and Onboarding
Submission Requirements (Must be provided before submission)
- Current Nursing License with primary source verification (within 30 days prior to start)
- Nursys license verification (within 30 days prior to start)
- Resume/Profile (Ascension Resume and Profile)
- At least two professional references
- Valid Driver's License or State ID (must not be expired and must match address on file)
Onboarding Requirements (Must be completed before or at start)
- Ascension Cerner Test-Out (Cerner test results and Training Checklist with signed attestation)
- Ascension Badge Photo (forward-facing headshot with white or blue background, no alterations)
- Health and Immunization Documentation:
- MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
- Hepatitis B: Vaccination series completion or signed declination
- Tdap: Valid within 10 years (no declinations accepted)
- Influenza Vaccine: Optional, but if declined during flu season (Oct 1 - Mar 31), mask required
- 2 Step PPD or IGRA testing with annual TB questionnaire
- Positive PPD requires negative chest X-ray within 1 year and annual TB questionnaire
- Fit Test: Required for all travelers, preferred before start (specific mask types required)
- Health Statement: Statement of good health or physical exam prior to first assignment, updated annually
- Drug Screening: Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually
- Background Checks and Verifications:
- Criminal Background Check including SSN Trace, Employment Verification, OIG Search, National Sex Offender
- EPLS (Excluded Parties List System) check within 90 days of start
- Employment Verification for past 7 years
- Education Verification relevant to clinical work within 90 days of start
- Skills and Competency Testing:
- Ascension Skills Checklist (clinical skills, updated annually)
- Ascension Specialty Exam (minimum 80% passing score, updated every 3 years)
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly)
- Ascension EKG Exam (minimum 80% passing score, updated as required)
- Ascension Color Vision Test (updated every 3 years)
- Certifications:
- BLS (AHA)
- ACLS (AHA)
- NIH Certification (Group A yearly, Groups B-F every 2 years)
- CPI/MOAB High Risk Online Portion (Nonviolent Crisis Intervention or MOAB De-Escalation Course prior to start)
- Onboarding Training:
- Ascension Nursing Contract Worker Onboarding (completed onsite)
- Ascension Onboarding Requirements Education Attestation (6-hour pre-hire training)
- Ascension Nova StatStrip Pre-Hire Modules (for Non-Allied Positions)
Optional Credentials
- Influenza Vaccine Exemption/Declination (mask required if declined during flu season)
- Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire (if applicable)
- Ascension Unit Specific Checklist (optional, uploaded by unit leader)
