Registered Nurse (MedSurg with ICU Experience) - Travel/Contract

Location: Port Clinton, Ohio
Contract Length: 13 weeks

• Shift: 7:00 PM - 7:30 AM
• Hours: 36 hours per week

Requirements

Licensure & Certifications

• Valid Registered Nurse license (Primary Source Verification required)
• ACLS (AHA Advanced Cardiac Life Support) required for all RNs
• BLS (AHA Basic Life Support) required for all travelers
• PALS (Pediatric Advanced Life Support) required if working in ER
• NRP (Neonatal Resuscitation Program) where applicable
• State License verification and background checks must be approved if any hits or license actions occur

Background Checks & Fingerprinting

• Criminal background check (7-year, FCRA compliant) required prior to start and must include:

  • SSN Trace
  • County, Statewide, Nationwide Criminal checks
  • National Sex Offender Registry
  • OFAC
  • FACIS Level 3
• Background check must be updated for breaks in service of 2+ months
• EPLS/GSA/SAM and HHS/OIG checks required and updated annually
• BCI & FBI fingerprinting required within 3 months prior to start; results must be received within 30 days of fingerprinting
• Fingerprint clearance attestation required
• Candidates may start with fingerprints pending if background check is complete

Health & Immunization

• Pre-employment physical exam within 1 year of start, confirming ability to safely perform job duties
• N95 fit test within 1 year of start
• Negative TB test (Quantiferon Gold or Tspot) within 1 year of start
• Immunizations or titers required within 1 year of start:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, vaccine series proof, or signed declination)
  • Varicella
• Tdap immunization within 10 years of start
• Negative 10-panel drug screen within 1 year of start, updated annually
• Seasonal flu vaccine documentation required during flu season (Oct 1 - Mar 31); exemptions accepted with agency approval

Competencies & Skills

• Unit-specific competency exam required, passing score needed, updated annually
• Unit-specific skills checklist required, updated annually
• Two positive professional references required, including at least one from the most recent employer
• Employment application must include 3 years prior work history with at least 2 years clinical practice in acute care and similar area

Additional Notes

• Radiation badge required for travelers assigned to Cath Lab units (not provided by Aya for new candidates)
• HIPAA training and orientation safety manual acknowledgment required
• Residency declaration form required if working on a compact license
