    Job Description

    • The RN Care Manager (RNCM) provides comprehensive care management including patient assessment, care planning, implementation, coordination, monitoring, and ongoing evaluation to ensure medically and financially appropriate care.
    • Responsibilities include facilitating interdisciplinary team care coordination and intensive medical management of participants recently discharged from acute care centers and skilled nursing facilities.
    • Collaborates closely with interdisciplinary team social workers and physicians to collect medical and social information to ensure safety and well-being of participants.
    • Primarily onsite work with occasional travel to participant residences for assessments (mileage reimbursed).
    • The RNCM starts from the center and travels to community visits.
    • The program focuses on keeping older adults healthy in the community through clinics, home visits, and more.
    • Candidates must be open to transitioning to permanent employment after the initial contract period.
    • Experience with the geriatric population is highly preferred.
    • All RNCMs are provided with a laptop and cell phone for work duties.

    Required Certifications & Licenses

    • Active Registered Nurse license in Washington (pending licenses not accepted)
    • ACLS (Advanced Cardiac Life Support) - AHA or Canadian Heart & Stroke Foundation certification
    • BLS (Basic Life Support) - AHA or Canadian Heart & Stroke Foundation certification
    • Other certifications may be required based on clinical track (e.g., PALS, NRP, Chemotherapy, EFM)
    • NURSYS verification required for all RNs
    • IV Certification (if applicable)
    • Management of Assaultive Behavior (MAB) (if applicable)
    • NIH Stroke Certification (if applicable)
    • Crisis Prevention Institute (CPI) (if applicable)

    Required Background Checks & Verifications

    • Background check within 45 days of start, covering all locations lived/worked in past 7 years, updated annually
    • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
    • Education verification for nursing degree (international education requires diploma/transcript)
    • State-specific searches including WA State Patrol Search and OBRA Registry Search (for CNAs only)
    • Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
    • References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty within the last 3 years

    Health & Immunization Requirements

    • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
    • Fit mask test (annual, travel only)
    • Tetanus/Diphtheria/Tdap vaccine within 10 years or declination
    • Negative TB test or chest X-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer with acceptable documentation
    • Hepatitis B vaccination series or signed declination
    • Varicella vaccination or positive titer with acceptable documentation
    • Seasonal flu vaccine or declination (required during flu season)
    • COVID-19 vaccine or declination (required if onsite)
    • Respiratory safety clearance (travel only, annual)

    Additional Requirements

    • Completion of Providence Health and Services WorkRamp training modules prior to start, including:
      • Core modules for caregivers
      • Specialty-specific modules if applicable
      • Annual regulatory modules
    • Photo for badge (passport style with neutral background) required
    • Employment application covering at least last 5 years (7 years for APN/subcontractors) with explanations for gaps or overlaps
    • Skills checklist updated annually (if applicable)

    Notes

    • This position is primarily onsite with some community travel.
    • Mileage reimbursement provided for travel to participant residences.
    • Locals and true travel candidates are welcome.
    • Candidates must be open to permanent placement after contract.
    • Experience with geriatric population is highly preferred.
    • No weekend or on-call requirements.
    • No floating between units.
