Job Opportunity: Home Health Physical Therapist (PT)
Location: Portland, Oregon
Specialty: Therapy/Rehabilitation
Employment Type: Travel/Contract
Contract Length: 26 weeks (approximately 6 months)
Job Description
- Provide skilled home health physical therapy services in East Portland and surrounding areas.
- Start Date: ASAP
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
- Patient Load: 1:1 ratio, may visit up to 6 patients per day; 30 productivity points standard
- Experience Required: Minimum 1 year of skilled home health experience
- First-time travelers accepted
- No weekend work required
- No on-call duties
- No floating between units
- Block scheduling not accommodated
- Locals accepted (within 50-75 mile radius) and considered local for pay purposes
- Pending license accepted (confirm with hiring manager regarding Return to Office restrictions)
- Guaranteed hours with possibility of being called off up to one shift every two weeks
Requirements & Certifications
- Certifications: Basic Life Support (BCLS) required
- Transportation: Must have own reliable transportation
- Vaccination: Primary COVID-19 vaccination series plus one booster required
- Documentation: Oasis experience required (please disclose if not experienced)
- Licensure: Must be licensed to work in Oregon; if working in border units, license for both Oregon and Washington may be required
- Background Checks: Comprehensive background checks required, including state-specific searches and verifications
- References: Minimum of 2 supervisor references covering relevant experience
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO
- Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
- Immunizations & Health:
- TDAP within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal Flu vaccine or declination (required during flu season)
- COVID-19 vaccine or declination
- Respirator Clearance: Required for travel candidates, must be medically cleared annually
Work Environment & Tools
- Facility Type: Home Health
- Attire: Business casual; scrubs allowed but not required (no jeans)
- Charting System: EPIC Remote Client (laptop), Rover app (phone), and Oasis documentation
- Parking: Free
- Mileage Reimbursement: IRS standard rates
Training & Compliance
- Completion of Providence Health and Services WorkRamp modules required prior to start, including:
- Core modules for Oregon home health professionals
- Specialty-specific modules as applicable
- Annual regulatory and compliance training
- Epic training modules required within one week of orientation
- Fit mask testing required annually (onsite if no recent record)
- Various background and education verifications required per Providence policies
Additional Notes
- Facility groups clients to minimize driving time; daily travel radius up to approximately 60 miles (60-90 minutes total drive time)
- Floating to other Providence facilities may be required in some cases (acknowledgement required)
- Locals within 50-75 miles are accepted and paid at the same rate as travelers
