Registered Nurse Travel/Contract Position

Location: Pleasant Prairie, Wisconsin
Contract Length: 13 weeks

Job Description

• Position is a travel/contract Registered Nurse role.
• Facility is part of a hospital system in Wisconsin.

Requirements & Compliance

Background Checks

• 7-year background check required within 1 year of start; updated annually.
• Must include SS trace, county searches for last 7 years of locations lived/worked/schooled, nationwide sex offender check.
• Wisconsin Caregiver Screening search required (completed online by QM or agency).

Certifications

• ACLS (Advanced Cardiac Life Support): ARC or AHA acceptable; not required for CNAs.
• BLS (Basic Life Support): ARC or AHA acceptable; not required for Medical Lab Technicians.
• PALS (Pediatric Advanced Life Support): ARC or AHA acceptable.
• NRP (Neonatal Resuscitation Procedure): As required.
• NIH Stroke Certification: Required for ICU, IMC/IMCU, inpatient, Med Surg, Step Down RN positions; must be renewed annually.
• Tech Cert Verification: Required as applicable based on licensure.

Competencies

• Medication Test/Pharmacology Competency Exam required as applicable.
• Color Blind Testing required for dialysis/kidney center roles (online test acceptable).
• Competency Exam required as applicable to profession/specialty.

Facility Specific Documents

• Photo for badge (passport-style, shoulders up, no filters).
• Compliance checklist to be completed by agency representative.
• Confidentiality statement to be signed prior to start.
• Flu vaccine religious or medical exemption forms required if applicable.
• Dosimeter request form required for Imaging Department only (not for MRI Tech).

Health Documents

• Physical exam required within 1 year of start (not annually required for non-clinical).
• Negative QGold TB test within 1 year of start (no TSpot/PPD accepted). Negative chest X-rays must be within 60 days of start.
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, series, or declination)
  • Varicella
• Drug testing: Negative 9-panel drug screen within 14 days before start; instant test acceptable.
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (Oct-Mar).

History & References

• Two professional references verified within 2 years of start.
• For inpatient RNs (MS, MS/TELE, IMC, Stepdown, ICU): one reference must be from most recent acute care assignment with performance comments.
• References must be completed prior to start or require VP approval, which may delay clearance.

Annual Requirements

• Skills checklist required annually per agency standards.
