    Job Description

    • 36 GWW Night shift

    Required Credentials and Certifications

    Personal Health History

    • COVID-19 Vaccine: Proof of vaccine and 1 booster or approved exemption (optional)
    • MMR: Proof of 2 immunizations or positive IGG titer; if negative, booster or declination accepted
    • Hepatitis B: Proof of 3 immunizations and positive antibody titer; if negative, booster or declination accepted
    • TB Test: Negative test within 30 days of start (skin test, QuantiFERON Gold, or T-spot); if positive, proof of history and chest x-ray within 5 years plus annual questionnaire
    • Health Statement: Signed statement within 1 year of start confirming ability to work without restrictions
    • Varicella: Proof of 2 immunizations, positive titer, or declination accepted
    • Tdap: Proof of vaccination within past 10 years or declination accepted
    • Covid Test: Required on start date for clinical staff (optional)
    • Influenza Vaccine: Required October - March unless exempted with medical or religious documentation
    • Drug Screen: 9-panel (minus THC) within 30 days of start, updated annually; rapid drug screen accepted

    Professional Credentials

    • Driver's License/State ID
    • CHS Profile: Resume or professional profile (required for submission)
    • 2 Professional References: Minimum 1 supervisory reference (required for submission)
    • Criminal Background Check: 7-year county and national check within 30 days of start, updated every 3 years
    • Social Security Trace: Optional if not included in background check
    • Employment Verification: Optional if not included in background check
    • OIG Search: Manual search within 30 days of start, updated annually
    • EPLS (Excluded Parties List System): Manual search within 30 days of start, updated annually
    • OFAC (Office of Foreign Assets Control): Manual search within 30 days of start, updated annually
    • National Sex Offender Search: Manual search within 30 days of start, updated annually
    • Medi-Cal Suspended and Ineligible Provider List: Search within 30 days prior to assignment and updated monthly thereafter
    • Confirmation of Assignment/Job Description: Must be updated with each extension
    • Acknowledgement of Receipt for Policy Packet: Must be signed and dated by candidate
    • Dispute Resolution Agreement: Must be signed and dated by candidate
    • Badge Photo: Neutral background, forward facing, no facial jewelry
    • ROI for MyChart: Must be initialed, signed, and dated by candidate
    • Any DNS (Do Not Rehire/Send): Attestation of any known record from prior 3 years on agency letterhead
    • Education Verification: Diploma, degree, or transcripts; board certification and primary source verification prior to first assignment and upon requalification

    Certifications

    • BLS: AHA or ARC only
    • ACLS: AHA or ARC only
    • NRP: Must be completed through AAP NRP learning platform
    • AWHONN/Fetal Heart Monitoring: Required once every 2 years

    Checklist/Testing

    • CHS Skills Checklist: Clinical skill-specific checklist completed within 1 year of start (required for submission)
    • HLC Confirmation: Testing to be completed by Friday before start date (optional)
