Job Opportunity: RN Cath Lab

Location: Columbus, Ohio
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Required Credentials & Certifications

• BLS: American Heart Association certification required for all direct patient care personnel.
• ACLS: Required for RNs in critical care areas including CCU, ED, PACU, MICU, SICU.
• NIHSS: Optional.
• Trinity Specialty Specific Competency Testing: Must be completed within the past year.

Licenses & Verifications

• License/Certificate Verification: Must be dated within 30 days of start. Online verification required.

Health & Immunization Requirements

• Fit Test: Required for onboarding (specific to mask type).
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray within last year or 5 years depending on region.
• COVID-19 Vaccine: Proof of vaccine or approved declination/exemption required (declination not accepted in CA or IL). Booster required through Sept 2025.
• Influenza Vaccine: Proof of vaccine or approved declination/exemption required (declination not accepted in CA or IL).
• Trinity 10 Panel Drug Screen: Completed within 30 days of start.
• MMR: Proof of 2 vaccinations or IgG titer required.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Varicella: Proof of vaccination or IgG titer required.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Trinity Color Vision: Required.

Professional & Compliance Requirements

• Trinity Profile: Resume, skills checklist (within a year), and 2 professional references required for submission.
• Trinity Code of Conduct Form: Must be signed and on file.
• Trinity I9: Standard I9 completed by agency.
• Confidentiality & Security Agreement: Must be signed and on file.
• Integrity and Audit Services Form: Signed within days prior to start.
• Conflict of Interest Disclosure Form: Must be signed and on file.
• Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic education verified by third party screening.
• EPLS & GSA/OIG Checks: Must be dated within past 30 days.
• Joint Commission CORE Exam: Must be completed within the past year.
• NSO Check: Must be dated within 30 days of start, including all aliases and locations.
• System Access Form: Must be completed and uploaded one week before start.
