Job Details

• Specialty Experience: Minimum 1 year hospital experience with GE scanners
• Prior Travel Experience: Not explicitly required

Certifications & Licenses Required

• BLS (AHA certified)
• ARRT CT certification
• State of Wisconsin license
• Current Driver’s License/State ID (must not be expired and address must comply with compact license rules)

Skills & Systems

• Experience with EPIC electronic health record system
• Ability to operate GE CT scanners

Health & Compliance Requirements

• MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
• Fit Test: Required annually; preferred before start, can be completed onsite
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD: Negative chest X-ray and annual TB questionnaire if applicable
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
• 2 Step PPD or IGRA: Required prior to start and annually thereafter
• Varicella: Proof of immunity or vaccination (no declinations accepted)
• Tdap: Valid within 10 years, no declinations accepted
• 12 Panel Drug Screen: Within 30 days prior to start and annually thereafter

Onboarding & Documentation Requirements

• Signed Assignment Agreement (required before onboarding)
• Ascension Resume/Profile and 2 Professional References (required for submission)
• License/Certificate Verification (current and primary source verified within 30 days prior to start)
• Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score; updated yearly)
• Ascension Medical Imaging Contract Worker Onboarding Education Attestation (6 hours pre-hire training attestation)
• Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender registry; updated annually)
• EPLS check (updated within 90 days of start)
• Skills Checklist (clinical, updated annually)
• Education Verification (relevant to clinical work, updated within 90 days of start)
• Employment Verification (past 7 years, updated annually)
• Confirmation of Assignment (start/end dates on agency letterhead)
