Job Opportunity: Therapy/Rehabilitation - Travel/Contract

Location: Los Angeles, California
Contract Length: 13 weeks (3 to 6 months assignment)
Shift Schedule:

• 5 days per week, 8-hour day shifts
• May include occasional weekend coverage
• Floating to a nearby facility 1-2 times per week

Requirements

• California License in hand
• Minimum 1 year of experience in Skilled Nursing Facility (SNF) setting
• New graduates will be considered
• Prior travel experience is not explicitly required

Certifications & Compliance

• ACLS (Advanced Cardiac Life Support) from American Heart Association or American Red Cross
• BLS (Basic Life Support) from American Heart Association or American Red Cross (current and clear copy required)
• PALS (Pediatric Advanced Life Support) preferred
• State License verification as applicable
• National Tech Certification as applicable (for tech roles)

Background & Health Documentation

• Government issued photo ID (Driver’s License or State ID)
• Emergency Contact Form prior to start
• Standard background checks including 7-year search, sex offender registry, and annual updates
• EPLS/GSA/SAM and HHS/OIG checks annually
• Physical exam within 1 year of start date
• Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
• Immunization or titer proof for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella (Chicken Pox)
• 9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)
• Seasonal Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)

• Competency Exam as applicable
• Skills Checklist required annually
• Professional References:

  • 2 for Travel, Allied, Per Diem Acute Care
  • 1 for Per Diem Non-Acute Care
  • Not required for Non-Clinical roles

Additional Notes

• 36 hours guaranteed per week
• Rates may vary based on experience
• Floating to a secondary location 1-2 times weekly may be required
• New graduates are welcome to apply, but minimum 1 year SNF experience preferred
