Compliance & Documentation Requirements
Background Checks
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Includes SSN Trace, Death Master Index, Nationwide and State Sex Offender checks, FACIS Level 3, Education Verification, MVR (if applicable), and international background if applicable
- Returning candidates with less than 30-day gap may not require new background check
- State-specific searches required for Washington and Oregon placements (not applicable here)
Health Documents
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Fit mask test (N95 or equivalent) required for travel starts after 2/5/2024; annual updates required
- Immunizations:
- TDAP (within 10 years or declination accepted)
- Negative TB test or chest X-ray within 12 months (annual updates required)
- MMR (2 vaccines or positive titer)
- Hepatitis B vaccination or signed declination
- Varicella (2 vaccines or positive titer)
- Flu vaccine required during flu season or declination accepted
- COVID vaccine or declination required
- Drug screen: 10-panel within 90 days of start (no instant tests)
Employment History & References
- Employment application covering at least last 5 years (7 years for APN/subcontractors)
- Minimum 2 references from supervisors covering 1 year of employment in specialty within last 3 years (6 months for CNAs)
Competency & Skills
- Medication/Pharmacology competency exam as applicable
- Skills checklist updated annually for applicable units
- EFM training required for Labor & Delivery RNs (via certification or assessment exam)
Training & Modules
- Completion of Providence-specific WorkRamp modules required prior to start, including:
- Core modules for RN profession in California
- Specialty-specific modules for Labor & Delivery and other applicable specialties
- Epic training modules (must be completed after Providence access granted)
- Annual regulatory and compliance modules
Additional Notes
- Float policy acknowledgment required; may float to other Providence facilities within reasonable distance
- Guaranteed hours: Facility may call off once per two-week pay period
- Badge photo required (passport style with neutral background)
- Local Rate Match applies within 50-mile radius
