Job Details
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Shift: PM shift (weekend flexibility required)
- Floating: Required to float between like units, specifically to mother-baby units and possibly to another nearby facility
- Local Radius: 50 miles
- Start Date: ASAP
- Weekend Requirement: Depends on length of contract
- Guaranteed Hours: Facility may call off once per two-week pay period
- Scrub Color: Any color allowed

Background Checks
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Must include: SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, MVR (if Home Health), and others as applicable
- Annual updates required
- Special state-specific searches for Washington and Oregon if applicable
Health Documents
- Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
- Fit Mask test (annual, travel only)
- Tetanus/Diphtheria/Tdap vaccine within 10 years or declination
- Negative TB test or Chest X-Ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer with declination option
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer with declination option
- 10 Panel Drug Screen within 90 days of start (no instant tests)
- Seasonal Flu Vaccine or declination during flu season
- COVID vaccine or declination
Competencies & Skills
- Medication Test/Pharmacology Competency Exam (as applicable)
- Unit-specific Skills Checklist (updated annually)
- Color Blind Testing (if required by unit)
- Competency Exams related to specialty (e.g., Fetal Heart Rate Monitoring Assessment for L&D RNs)
Facility Specific Requirements
- Must complete Providence Health and Services WorkRamp Core and Specialty Modules prior to start
- Epic training modules required (completion and transcript submission)
- Photo for badge (passport style with neutral background)
- Floating Policy Acknowledgement (may be required to float to other Providence facilities within reasonable distance)
References & Employment History
- Minimum of 2 supervisor references covering 1 year of employment within the last 3 years in a comparable specialty/setting
- Employment application covering at least last 5 years (7 years for APN/Subcontractors) with month/year format for dates
- Clarification required for any gaps or overlaps in employment
Additional Notes
- Must be flexible to work weekends and float between units as needed
- Precepting skills preferred but not mandatory
- Local candidates within 50 miles accepted
- No pending licenses accepted
