Location: Fresno, California

- Varicella: Proof of 2 immunizations, positive titer, or declination accepted
- Tdap: Proof of vaccination within past 10 years or declination accepted
- Influenza Vaccine: Required October - March unless medically or religiously exempted
- COVID Test: Required on start date for clinical staff (optional)
- Drug Screen: 9 panel (minus THC), within 30 days of start, updated annually; rapid drug screen accepted
Professional & Background Checks
- Driver’s License or State ID (passport accepted)
- 7-year County and National Criminal Background Check, SSN Trace, Employment Verification (5 years), OFAC, NSO, and other federal exclusions (updated every 3 years; must be within 30 days of start)
- Manual searches for OIG, EPLS, OFAC, National Sex Offender Registry within 30 days of start and updated annually
- CA Medi-Cal Suspended and Ineligible Provider List check within 30 days prior to assignment and updated monthly
- CHS-specific background checks and documentation including professional references and education verification
Additional Requirements
- Health Statement: Signed and dated within 1 year of start, stating ability to work without restrictions
- Skills Checklist: Clinical skill-specific checklist to be completed within 1 year of start
- Various CHS policy acknowledgments, dispute resolution agreements, and confidentiality receipts required on or before start date
- Badge photo with specific guidelines (neutral background, no facial jewelry)
