Location: Fresno, California

Job Details

Immunizations and Health Requirements

- MMR: Proof of 2 immunizations or positive IGG titer; if negative titer, booster or declination accepted.
- Hepatitis B: Proof of 3 vaccinations, positive antibody titer, or declination accepted; if negative titer, booster or declination accepted.
- TB Test: Negative TB skin test, QuantiFERON Gold, or T-spot within 30 days of start; if positive, proof of positive history and chest x-ray within last 5 years required.
- Varicella: Proof of 2 immunizations, positive titer, or declination accepted.
- Tdap: Proof of vaccination within past 10 years or declination accepted.
- Influenza Vaccine: Required from October to March unless medically or religiously exempted.
- Drug Screen: 9 panel (minus THC), within 30 days of start, updated annually; rapid drug screen accepted.
Background and Compliance Checks
- 7-year County and National Criminal Background Check, SSN Trace, Employment Verification (5 years), OFAC, NSO, and other federal/state exclusion lists within 30 days of start; updated every 3 years.
- CHS OIG, EPLS, OFAC, National Sex Offender, and Medi-Cal Suspended and Ineligible Provider List checks required within 30 days of start and updated monthly or annually as specified.
- CHS Driving Record (optional if run with background, otherwise must be updated annually).
- CHS Any DNS (Do Not Rehire/Send) attestation from prior 3 years.
Additional Requirements
- Health Statement: Signed and dated within 1 year of start, stating ability to work without restrictions; updated annually.
- Resume/Profile submission required.
- Two professional references required, including at least one supervisory reference.
- Skills Checklist: Clinical skill-specific checklist to be completed within 1 year of start.
- Various CHS policy acknowledgments, dispute resolution agreements, confidentiality receipts, and assignment confirmations required.
- Badge photo with specific guidelines (neutral background, no facial jewelry).
This position requires compliance with extensive credentialing, immunization, and background verification protocols to ensure patient safety and regulatory adherence.
