- Varicella - proof of immunity or vaccination
- Seasonal Flu vaccination during flu season (Nov-April) or signed declination
- Physical exam within 1 year of start, must clear for work
- Fit mask test within 1 year of start, renewed annually (exceptions apply)
- Respiratory safety clearance required prior to start, renewed annually (exceptions apply)
Work History & Documentation
