• Physical/Health Statement (within last 12 months; must include vitals, Snellen eye chart, and physician approval)

Professional Requirements

• Agency Drug Screen (within 30 days of start; specific drug panel required; marijuana testing prohibited by NY state)

• 2 Professional References (1 supervisor reference within last 12 months, 1 supervisor or peer reference within last 3 years)
