Job Description
- Open PRN order for RN ECMO.
Required Credentials
- BLS Certification (AHA or ARC Only) - Required for onboarding.
- COVID-19 Vaccine - Proof of vaccine and 1 booster or exemptions must be submitted for approval (optional).
- MMR Immunization - Proof of 2 immunizations or positive IGG titer; if negative, must have booster or declination accepted - Required for onboarding.
- Fit Test - Will be completed onsite on start date (optional).
- Hepatitis B Immunization - Proof of 3 immunizations and positive antibody titer; if negative, booster or declination accepted - Required for onboarding.
- Influenza Vaccine - Required from October to March unless otherwise indicated; medical/religious exemptions must be approved - Required for onboarding.
- TB Test - Must be within 30 days of start; negative results required - Required for onboarding.
- Health Statement - Statement signed and dated within 1 year of start stating traveler is able to work without restrictions - Required for onboarding.
- Driver's License/State ID - Required for onboarding.
- Varicella Immunization - Proof of 2 immunizations or positive titer or declination accepted - Required for onboarding.
- Tdap Immunization - Proof of vaccination within past 10 years or declination accepted - Required for onboarding.
- Covid Test - Required on start date for clinical staff (optional).
- Resume or Profile - Required for submission.
- Professional References - 2 references required, with at least 1 being supervisory.
- Criminal Background Check - Must be within 30 days prior to start; updated every 3 years thereafter.
- OIG, EPLS, OFAC, and National Sex Offender Searches - Must be completed within 30 days of start and updated annually.
- Medi-Cal Suspended and Ineligible Provider List - Must be completed within 30 days prior to first assignment and updated with each extension.
- Drug Screen - Must be within 30 days of start; updated annually.
- Skills Checklist - Required to be completed within 1 year of start for clinical staff.
- Confirmation of Assignment/Job Description - Must match details from the order/assignment tab.
- Acknowledgement of Receipt for Policy Packet - Must be signed and dated by candidate.
- Dispute Resolution Agreement - Must be signed and dated by candidate.
- Badge Photo - Must meet specified requirements.
- Any DNS (Do Not Rehire/Send) - Attestation required from agency.
- HLC Confirmation - Testing to be completed by the Friday before start.
- Confidentiality of Receipt - Must be signed and dated by candidate.
- License/Certificate Verification - Primary source verification required within 30 days of start.
