Job Opportunity: Physical Therapist (PT) - Skilled Nursing Facility (SNF)
Location: Lancaster, California
Employment Type: Travel/Contract
Contract Length: 13 weeks (3 to 6 months assignment)
Job Details
- Specialty: Therapy/Rehabilitation (Physical Therapy in SNF setting)
- Shift: 5 days per week, 8-hour day shifts
- Schedule Notes: May include occasional weekend coverage and floating as needed
- Start Date: April
Experience Requirements
- Minimum 1 year of experience in Skilled Nursing Facility (SNF) specialty preferred
- New graduates will be considered
- Prior travel experience is NOT explicitly required
Certifications & Licenses Required
- Current State License verification (primary source online verification)
- BLS (Basic Life Support) from American Heart Association or American Red Cross
- ACLS (Advanced Cardiac Life Support) preferred
- PALS (Pediatric Advanced Life Support) preferred
- National Tech Certification as applicable (for tech roles)
Compliance & Documentation Requirements
- Government issued photo ID (Driver’s License or State ID)
- Emergency Contact Form prior to start
- Background checks including:
- 7-year work/school/location history
- EPLS/GSA/SAM verification (annual)
- HHS/OIG verification (annual)
- Competency exams as applicable by profession
- Physical exam within 1 year of start date
- Negative TB test or Chest X-Ray within 1 year (with specific rules for past positives)
- Immunization or titer proof for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella (Chicken Pox)
- 9 Panel Drug Screen within 1 year of start
- Seasonal Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)
References & Skills
- 2 professional references required for travel, allied, or per diem acute care roles
- 1 professional reference required for per diem non-acute care roles
- Annual skills checklist required
This position is ideal for a Physical Therapist with at least 1 year of SNF experience looking for a travel contract in California with a flexible start date and a 3 to 6 month assignment length. New graduates are welcome to apply.
