    Job Opportunity: Registered Nurse (Home Health)

    Location: Alameda, California
    Employment Type: Travel/Contract
    Contract Length: 13 weeks


    Job Description

    • Provide direct patient care under the supervision of the Clinical Manager and Primary Care Clinician.
    • Implement physician plans of care and obtain physician orders.
    • Supervise home health aides and coordinate care within an interdisciplinary team.
    • Participate in utilization management and control of patient supplies.
    • Deliver care tailored to the age-related, developmental, psychosocial, environmental, educational, and communication needs of patients.
    • Accountable for ensuring the plan of care is followed according to professional standards and agency protocols.
    • On-call as needed; no floating or weekend shifts required.

    Required Certifications

    • California Registered Nurse (CA RN) license
    • AHA Basic Life Support (BLS) certification
    • Advanced Cardiac Life Support (ACLS) - AHA or ARC only (verify by position)
    • Pediatric Advanced Life Support (PALS) - AHA or ARC only (verify by position)
    • Neonatal Resuscitation Program (NRP) - AHA/AAP only, must be Instructor or Advanced Provider Course (verify by position)
    • Other certifications may be required depending on position (e.g., CCRN, TNCC, EFM, CPI)

    Background Checks & Verifications

    • 7-year background check required within 6 months of start, including SSN trace with all aliases, county searches, sex offender search, education verification, and motor vehicle report (for driving positions).
    • FACIS Level 3 screening required within 2 weeks of start and monthly thereafter.
    • EPLS/GSA/SAM, HHS/OIG, OFAC sanctions list, and Medi-Cal Suspended and Ineligible Provider List checks as applicable.
    • Education verification required (Bachelor of Science in Nursing preferred).
    • State license primary source verification within 30 days of start.
    • NURSYS verification within 30 days of start.

    Health & Immunization Requirements

    • Physical exam within 1 year of start (must indicate cleared for work).
    • Fit test within 1 year of start, renewed annually (exceptions apply).
    • Immunizations and/or titers required:
      • Tetanus/Diphtheria/Tdap (within 10 years)
      • Negative TB test or chest x-ray within 90 days, renewed annually
      • Rubeola (Measles), Mumps, Rubella (MMR) immunity or vaccination
      • Hepatitis B series or declination
      • Varicella immunity or vaccination
      • Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
    • Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per CA law).

    Competency & Skills Requirements

    • Medication/Pharmacology competency exam as applicable.
    • Unit-specific skills checklist required, renewed annually.
    • Dysrhythmia/EKG competency exam may be required for some units.
    • Pass/fail testing (e.g., EKG/medication) may be required for ICU RNs at certain locations.

    Employment History & References

    • Full 7 years of work history required on application with no gaps.
    • Employment verification for up to 7 years required; gaps over 30 days must be explained.
    • Two professional references required for clinical positions (within 3 years).

    Additional Information

    • Personal or auto insurance required for home health and driving positions, effective for the duration of the assignment.
    • Candidates must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, and regulatory orientation prior to start.
    • TB screening questionnaire required within 90 days of start and renewed annually.
    • Religious exemption forms available for certain vaccine and PAPR requirements; must be approved by Aya Healthcare.
