    Required Certifications

    • AART (American Association of Radiologic Technologists)
    • BLS (Basic Life Support)
    • ACLS, PALS, NRP, CHEMO, EFM, IV Certification, MAB, NIH Stroke Certification, CPI, and others may be required depending on clinical track and specialty verification. Always verify certifications with clinical leadership or credentialing system.
    • State License: CPDH Certificate required for all Respiratory Therapists working in California.
    • NURSYS Verification required for all RNs.
    • Additional certifications may be required based on specialty and facility requirements.

    Compliance & Background Requirements

    • Background Check within 45 days of start, covering all locations lived/worked in the past 7 years. Must be updated annually.
    • E-Verify attestation required.
    • Education Verification required for all candidates.
    • Drug Testing: 10 Panel Drug Screen within 90 days of start (no instant tests).
    • Physical Exam within 1 year of start, signed by MD, NP, PA, or DO.
    • Immunizations and Health Documents:
      • TDAP (within 10 years) or declination accepted
      • Negative TB test or Chest X-Ray within 12 months, updated annually
      • MMR (Measles, Mumps, Rubella) vaccination or positive titer
      • Hepatitis B vaccination or signed declination
      • Varicella vaccination or positive titer
      • Flu vaccine required during flu season or declination accepted
      • COVID vaccine or declination required
    • Respirator Safety Clearance required for travel candidates, updated annually.
    • Fit Mask testing required for travel candidates (except Radiation Therapists at this facility).

    Documentation & History

    • Employment Application covering at least last 5 years (7 years for APN/Subcontractors).
    • Minimum of 2 references covering 1 year of employment within last 3 years (6 months for CNAs). References must be from supervisors.
    • Skills Checklists as applicable, updated annually.
    • Radiation Badge issued by facility if applicable.

    Facility Specific Requirements

    • Completion of Providence Health and Services WorkRamp modules:
      • Core Regulatory Modules (annual)
      • Specialty Specific Modules (for ER, L&D, Leadership, Hospice/Home Health/Assisted Living)
      • Regional Core and Specialty Modules based on location and specialty
    • Epic Access training modules required for certain regions and specialties.
    • Photo for badge with neutral background required.
    • Float Policy Acknowledgement: May be required to float to other Providence facilities within reasonable distance.
