Job Opportunity: RN - Case Management
Location: Gilbert, Arizona
Employment Type: Travel
Contract Length: 13 weeks
Facility Overview
- 40-bed inpatient acute rehabilitation hospital
- Focus on treatment and recovery for patients with stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury
- Comprehensive physical medicine and rehabilitation programs
Position Details
- Role: RN Case Manager
- Specialty: Case Management
- Shift: 8am - 4pm
- Weekly Hours: 40
- Patient Ratio: 1:13
- EHR Systems: Cerner, UDS
- Patient Population: General
Required Qualifications
- Current Registered Nurse (RN) licensure
- Certification in Case Management or Rehabilitation preferred (e.g., Commission for Case Manager Certification (CCM), Association of Rehabilitation Nurses (ARN), American Case Management Association (ACM), or Board Certification in Case Management by ANCC such as RN-BC)
- Minimum of 2 years of social work or case management experience in an inpatient setting strongly preferred
- Acute/rehabilitation hospital experience preferred
- Basic computer skills and effective communication in English (verbal and written)
- Education: Master’s or Bachelor’s degree in Social Work
Preferred Experience & Skills
- Previous inpatient rehabilitation experience strongly preferred
- Experience with stroke and hip fracture patient populations
- Cooperative team player personality
