Description

Job Opportunity: Physical Therapist (PT) - Home Health

Location: Oakland, California (Solano County Territory)
Employment Type: Contract
Contract Length: 13 weeks

Role Responsibilities

Prioritize start of care visits
Supervise Physical Therapy Assistants
Mandatory next day reporting of start of care visits
Complete clinical notes and documentation within 48 hours of visit
Maintain timely communication with supervisor, intake, and office
Be generally responsive during normal business hours

Schedule

40 hours per week
Workdays: Saturday through Wednesday

Training

3 full days (1 in-house, 2 in the field)

Productivity Requirements

30 points per week (1 point = 1.33 hours)
Caseloads and visit mixes average 8 hours per day, 40 hours per week
Overtime requires pre-approval by Clinical Supervisor

Start of Care: 2 points
Recertification, Resumption of Care, OASIS Discharge, Evaluation: 1.5 points
Revisit, NON OASIS Discharge: 1 point

Required Qualifications

California Physical Therapist (PT) License
Minimum 1 year of Home Health experience
