Required Qualifications

  • Specialty: Radiology / Cardiology (CT Tech)

  • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO
  • Immunizations & Health Documents:
    • TDAP (Tetanus/Diphtheria/Pertussis) within 10 years or declination
    • Negative TB test or chest X-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer
    • Flu vaccine during flu season or declination
    • COVID vaccine or declination
  • Fit Mask Test: Required for travel candidates, updated annually
  • Respiratory Safety Clearance: Required annually for travel candidates
  • References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
  • Employment Application: Must include at least last 5 years of employment with month/year format, gaps explained
  • Skills Checklist: Required for applicable units, updated annually

Facility & System Specific Requirements

  • Must complete Providence Health and Services required WorkRamp training modules prior to start, including:
    • Annual Regulatory Requirements
    • Specialty Specific Modules (if applicable)
    • Regional Core and Specialty Modules for Oregon
  • Epic training modules required for charting system access
  • Photo for badge: Passport style with neutral background required
  • Float Policy Acknowledgement: Required (may be required to float to other Providence facilities within reasonable distance)
