Job Summary

Seeking a CT Technologist for weekend night shifts (17:30 - 06:00).

Required Qualifications

• Valid CT Technologist license in Ohio
• Minimum of 1 year of CT Technologist experience within the last year

Health and Compliance Requirements

• COVID-19 Vaccine (including booster by Sept 2025) or approved exemption.
• MMR vaccination proof or immunity.
• Fit Test required.
• Hepatitis B vaccination series initiated prior to start (titers not accepted).
• Influenza vaccine or approved exemption for 2024-2025 season.
• TB screening: 2 Step PPD or IGRA with chest x-ray if positive (within last 5 years or 1 year depending on region).
• Varicella vaccination proof or immunity.
• Physical/Health Statement confirming fitness to work (exam within last year).
• 10 Panel Drug Screen completed within 30 days of start (no THC).

Professional and Background Requirements

• Resume and skills checklist within the past year.
• Confidentiality and proprietary information agreement.
• Conflict of interest disclosure.
• Criminal background check (federal, nationwide, county for last 7 years).
• Education verification (highest academic degree or diploma/GED).
• EPLS (Excluded Parties List System) check within past 30 days.
• OIG (Office of Inspector General) exclusion check within past 30 days.
• Integrity and audit services form signed prior to start.
• Joint Commission CORE exam within the past year.
• NSO (National Sex Offender) check within past 30 days.
• System access form completed one week before start.
• Specialty specific competency testing completed within the past year.
• Code of conduct form signed.
• Standard I9 verification completed by agency.
• Color vision test.

Additional Notes

• Agency will provide a skills checklist for clinical contractors.
• All documentation must be uploaded into the agency’s system prior to onboarding.
