Job Opportunity: Physical Therapist (PT) - Home Health

Location: Oakland, California
Employment Type: Contract
Contract Length: 13 weeks

Role Overview

• Prioritize start of care visits
• Supervise Physical Therapy Assistants
• Mandatory next-day reporting of start of care visits
• Complete clinical notes and documentation within 48 hours of visit
• Maintain timely communication with supervisor, intake, and office
• Be generally responsive during normal business hours

Schedule

• 40 hours per week, Monday to Friday
• One weekend per month rotation

Training

• 3 full days (1 in-house, 2 in the field)

• Verizon One Talk

Productivity Requirements

• 30 points per week (1 point = 1.33 hours)
• Caseload averages 8 hours per day, 40 hours per week
• Overtime requires pre-approval

Visit Types and Point Values

• Start of Care: 2 points
• Recertification, Resumption of Care, OASIS Discharge, Evaluation: 1.5 points
• Revisit, Non-OASIS Discharge: 1 point

Required Qualifications

• California Physical Therapist (PT) License
• Minimum 1 year of Home Health experience

• Territory includes Sacramento, CA, with possible expansion
• Mileage, parking, and toll reimbursement provided
• Company car option available (with adjusted pay)
