Description
Job Opportunity: Physical Therapist (PT) - Home Health
Location: Oakland, California
Employment Type: Contract
Contract Length: 13 weeks
Role Overview
- Prioritize start of care visits
- Supervise Physical Therapy Assistants
- Mandatory next-day reporting of start of care visits
- Complete clinical notes and documentation within 48 hours of visit
- Maintain timely communication with supervisor, intake, and office
- Be generally responsive during normal business hours
Schedule
- 40 hours per week, Monday to Friday
- One weekend per month rotation
Training
- 3 full days (1 in-house, 2 in the field)

Productivity Requirements
- 30 points per week (1 point = 1.33 hours)
- Caseload averages 8 hours per day, 40 hours per week
- Overtime requires pre-approval
Visit Types and Point Values
- Start of Care: 2 points
- Recertification, Resumption of Care, OASIS Discharge, Evaluation: 1.5 points
- Revisit, Non-OASIS Discharge: 1 point
Required Qualifications
- California Physical Therapist (PT) License
- Minimum 1 year of Home Health experience
Additional Notes
- Territory includes Sacramento, CA, with possible expansion
- Mileage, parking, and toll reimbursement provided
- Company car option available (with adjusted pay)
