Travel RN Medical/Surgical Position

Location: Willoughby, Ohio
Contract Length: 13 weeks

Required Experience

• Positive PPD: Clear chest x-ray and annual TB questionnaire required; chest x-rays valid for 5 years
• Fit Test: Completed within 1 year of start; specific mask types accepted

Background & Verification Requirements

• Criminal Background Check: Must be completed within 6 months prior to start; includes Social Security Trace, County searches, National Criminal Sweep, and National Sex Offender check
• Ohio Fingerprints: Required for Ohio residents; includes BCI&I check and FBI fingerprint check depending on residency duration
• Education Verification: Highest level of education verified through third-party; international education requires equivalency documentation
• Employment Verification: Verification of employment within last 5 years through third-party; attestations not allowed

Testing & Competency Requirements

• Drug Screen: 10-panel drug screen completed within 60 days of start; instant drug screens not accepted
• Competency Exam: Must score at least 80%; completed within 1 year prior to start
• Core Competencies: Includes HIPAA, Joint Commission-NPSG, OSHA topics; 80% passing score required; completed within 1 year prior to start
• Skills Checklist: Clinical skill-specific checklist completed within 1 year prior to start (initial hire only)

Additional Requirements

• Physical/Health Statement: Pre-employment physical exam within 1 year of start stating ability to work without restrictions
• UHS I-9 Attestation: Proof of eligibility to work required
• UHS Badge Photo: Recent headshot required
• Driver's License/State ID: Non-expired ID required
• Submission Packet: Resume, professional references (within last 2 years), job application, and employment history form required
