Description
Job Opportunity: RN - Case Management (Travel)
Location: Indianapolis, Indiana
Employment Type: Travel Contract
Contract Length: 4 weeks
Job Description
- Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
- Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
- Enhance patient management quality and satisfaction, promoting continuity of care and cost-effectiveness.
- Integrate case management, utilization review, and discharge planning functions.
- Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
- Collaborate strongly with interdisciplinary teams (IDT).
Required Qualifications
- License: Indiana RN license or multistate RN license.
- Certifications: BLS certification through the American Heart Association.
- Experience: Minimum 2 years recent RN Case Management experience.
- Experience must include work in LTAC or STACH settings, MCG guidelines, and discharge planning across acute care settings.
- Prior travel experience is not explicitly required.
Schedule
- Monday through Friday, 8:00 AM to 4:30 PM
Additional Information
- Strong interdisciplinary team collaboration required.
- Facility setting includes long-term acute care and short-term acute care hospital environments.
