Description
Job Opportunity: Registered Nurse Case Manager
Location: Sitka, Alaska
Employment Type: Contract
Contract Length: 13 weeks
Job Summary
- Manage patient care actively within a multidisciplinary team.
- Collaborate with medical providers and clinical staff to establish and achieve individualized medical goals.
- Coordinate patient referrals to specialty clinics and track outcomes.
- Serve as primary clinical contact for patient care and care coordination.
- Perform nursing duties including assessment, documentation, education, and medication administration.
- Maintain safety, infection control, and annual competencies.
- Foster teamwork and participate in department improvement efforts.
Required Qualifications
Education
- Graduate of an accredited school of nursing (Bachelor’s degree preferred)
- High school diploma or GED
Experience
- Minimum 1 year of case management experience
Licensure & Certification
- Active, unrestricted Registered Nurse (RN) license in the State of Alaska
- Basic Life Support (BLS) certification
Additional Requirements
- Must have flu vaccination
- Must live at least 50 miles away from the facility location
- Ability to start no less than 3 weeks after fingerprint cards are received
Skills & Abilities
- Knowledge of nursing process, customer service, safety, and infection control principles
- Strong assessment, communication, and data collection skills
- Ability to multi-task and work independently in a fast-paced environment
- Team player with willingness to teach and ask for help when needed
- Critical thinking and problem-solving skills
- Ability to work within a theory-based nursing practice model and evidence-based practices
Prior Travel Experience
