Care Coordinator Nurse (Case Manager)
Location: Fort Defiance, Arizona
Employment Type: Contract
Contract Length: 13 weeks
Job Description
- Collaborate with nursing staff and providers to support patient care within an outpatient clinic setting.
- Work schedule: Monday through Friday, 8:00 AM to 5:00 PM (no weekends or holidays).
- Occasional floating to other departments may be required, performing tasks such as patient care or support duties depending on experience and competency.
Required Qualifications
- Specialty: Nursing
- Required Years of Experience: 5 years
- License: Registered Nurse (RN) license required in the state of Arizona.
- Certification: BLS/BCLS certification required.
Additional Information
- Orientation occurs every other Monday; missing the compliance deadline will delay orientation to the next scheduled date.
