Job Opportunity: Home Health Case Manager RN

Location: Windom, Minnesota
Contract Length: 12 weeks

Job Summary

Seeking a Home Health Case Manager RN to provide skilled nursing care in a home health setting. This role involves working with patients in their homes, managing care plans, and ensuring quality patient outcomes.

Work Schedule

• Guarantee: 36 hours per week for 12-hour shifts, or 40 hours per week for 8-10 hour shifts
• Work week: Sunday through Saturday
• Typical hours: Monday through Friday, 8:00 AM to 4:30 PM
• On-call: Every 4th week on call, Monday-Friday 4:30 PM to 8:00 AM, and Friday 4:30 PM through Monday 8:00 AM
• Every 4th holiday on call

Required Qualifications

• Current RN license valid in Minnesota
• Minimum 1 year of RN experience
• BLS/CPR Certification required
• Experience with Netsmart EMR system
• Proficiency in OASIS completion

Additional Information

• No on-call or call-back available outside scheduled on-call weeks
• Local placements within an 80-mile radius
• If housing is used, local rates apply
• A $500 fee applies if clinician backs out or quits with less than 14 days' notice
