Job Opportunity: Case Manager (Registered Nurse)
Location: Novato, California
Employment Type: Travel/Contract

Job Summary
Seeking an experienced Registered Nurse with home health experience as a case manager (not hospice or hospital case management). The role involves overall management of patients in a home health setting, including direct patient care, patient/family education, and coordination of interdisciplinary care teams. The position requires planning, organizing, and directing patient care services to ensure quality and compliance with standards and regulations.
Key Responsibilities
- Conduct comprehensive patient and family assessments covering physiological, spiritual, and emotional needs.
- Collaborate with physicians and healthcare team members to develop and adjust patient-centric in-home care plans.
- Coordinate interdisciplinary teams to implement and manage patient care plans effectively.
- Provide compassionate, evidence-based nursing care in accordance with policies and standards.
- Educate patients and families on care plans and treatments, ensuring understanding through teach-back methods.
- Maintain patient confidentiality and comply with all applicable laws and regulations.
- Monitor and evaluate patient care outcomes, adjusting plans as necessary.
- Document all patient care activities accurately in the electronic health record (EHR).
- Participate in safety event reporting and analysis to improve patient safety.
Required Qualifications
- Valid Registered Nurse (RN) license in California.
- Minimum of 1 year of case management experience, with case management experience within the last year.
- Experience in a home health setting.
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Proficiency with Epic electronic health record system.
- Experience with OASIS documentation.
Background and Compliance Requirements
- 7-year background check including SSN trace, county searches, sex offender, education verification, and motor vehicle report (for driving positions).
- FACIS Level 3 screening and other federal/state required checks updated as per policy.
- Education verification (Bachelor of Science in Nursing preferred).
- Drug screening (9-panel, excluding marijuana/cannabinoids due to California law).
- Physical exam within 1 year of start.
- Immunizations and health screenings including:
- TB test or chest x-ray (annual renewal required).
- Tetanus/Diphtheria/Tdap (within 10 years).
- Measles, Mumps, Rubella (MMR) immunity or vaccination.
- Hepatitis B vaccination or declination.
- Varicella immunity or vaccination.
- Seasonal flu vaccination during flu season or approved declination.
- Fit mask test (annual renewal).
- Respiratory safety clearance (annual renewal).
- Completion of facility-specific regulatory and compliance documents prior to start.

- Must have full 7 years of verifiable work history with no significant gaps.
- References required per agency policy (not required by client).
- Must review and sign all required facility-specific policies and attestations prior to start.
