Job Opportunity: Registered Nurse - Case Management

Location: Fort Wayne, Indiana
Employment Type: Travel Contract
Contract Length: 13 weeks

Job Description

Position involves care management with a focus on discharge planning experience in a hospital setting.
Patient ratio is approximately 1:20.
Work shift: 8:00 AM to 4:30 PM, including holidays that fall during the contract period.
Dress code: Black scrubs or business casual.
Electronic Medical Record (EMR) system used: Cerner.

Required Qualifications

Valid RN license in Indiana.
Minimum of 4 years of case management experience, including acute hospital case management.
Case management experience within the last year.
Discharge planning experience in hospital setting care management.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Additional Information

Position may require holiday shifts.
No floating to other units specified.
Prior travel experience is not explicitly required.
