Job Opportunity: Registered Nurse (Home Health / Hospice)

Location: Beaver Dam, Wisconsin
Contract Length: 13 weeks


Job Description

  • Provide home health and hospice nursing care primarily for a geriatric population.
  • Patient diagnoses include: Heart Disease, Chronic Respiratory Diseases, CHF, COPD, Pneumonia, Dementia/Alzheimer’s, Depression, Stroke, Cancer, Kidney Disease, Diabetes, Liver Disease, Wounds, Joint Replacement, Surgery, Medication Management.
  • Patient load: Approximately 5-6 patients per day, 25-30 points per week, including routine and OASIS visits.
  • Duties include both home health and hospice visits.
  • Participate in daily 15-minute huddle calls, weekly interdisciplinary team meetings, and monthly all-staff meetings.
  • No ventilator care; occasional CPAP & BiPAP patients who self-manage.
  • Utilize critical thinking and chronic disease management skills.
  • Provide patient and family education.
  • Skilled in Negative Pressure Wound Therapy (NPWT), ostomy care, central line care, IV administration, and complex wound care.
  • Hospice care includes pain and symptom management, end-of-life care, empathy, and compassion.

Schedule & Work Environment

  • Work hours: Monday through Friday, 8:00 AM to 4:30 PM (end time may vary).
  • Weekend work required only during on-call shifts.
  • Must be available to work at least one major holiday and one minor holiday.
  • On-call requirement: One on-call week/weekend every 6 weeks (days do not have to be consecutive).
  • No floating between departments, but will cover both home health and hospice visits.
  • Dress code: Solid color scrubs (no patterns or logos unless company-issued), closed-toe non-slip shoes with socks or hosiery. No flip-flops, open-toe shoes, or sandals allowed in patient care areas.

Orientation & Training

  • 1 day of computer-based training.
  • 2-5 days of field training/shadowing depending on experience to learn department-specific procedures.

Required Qualifications

  • Current Registered Nurse (RN) license.
  • BLS (Basic Life Support) certification from the American Heart Association.
  • Experience in home health and hospice nursing.
  • Prior travel experience is required.
  • Skills in critical thinking, chronic disease management, patient/family education, NPWT, ostomy care, central line care, IV administration, and complex wound care.
  • Hospice experience including pain and symptom management and end-of-life care.

Additional Information

  • Certified nursing assistants support the team.
  • Administrator available after hours, weekends, and holidays.
  • Clinicians must clock in and out at the facility.
  • Candidates must disclose if they have previously worked for this employer; if so, there must be a minimum 6-month gap since last employment.
