Job Description / Responsibilities
- Perform assessments, evaluations, discharge planning, and documentation including long-term and short-term goals, plan of care, and reassessments.
- Work within acute care, outpatient clinic, and skilled nursing facility (SNF) settings (preferred).
- Use equipment such as bed alarms, call lights, crutches, gait belts, hospital beds, Hoyer lifts, prosthetics (upper and lower extremity), rolling walkers, shoulder slings, sit-to-stand lifts, sliding boards, standard walkers, supplemental oxygen, and traction tables.
- Conduct assessments including body mechanics, pain assessment, strength testing, and vital signs.
- Patient populations primarily geriatrics and orthopedics.
- Floating within the service line may be required to meet facility needs.
Required Qualifications
- Valid Physical Therapist license in Missouri.
- Minimum 1 year of Physical Therapist experience within the last year.
- Certification from NBSTSA or NCCT.
Special Requirements
- Experience with swingbed (skilled) care, inpatient acute, and outpatient rehab services.
Compliance & Documentation Requirements
- Government-issued ID required.
- Criminal background check within 6 months of start, including statewide and nationwide searches.
- Quarterly EPLS/GSA/SAM and HHS/OIG checks.
- Education verification of highest degree related to Physical Therapy.
- Professional license primary source verification.
- Drug testing within 1 year of start, including a comprehensive panel.
- Physical exam within 1 year of start.
- Fit mask (N95) testing required and must be renewed annually.
- Immunizations and titers required or declination forms for:
- Tetanus/Diphtheria/Tdap (every 10 years)
- Measles (Rubeola)
- Mumps
- Rubella
- Varicella
- Negative TB test or chest x-ray within 1 year of start.
- Seasonal flu vaccination or approved exemption required during flu season.
- Various facility-specific compliance documents including HIPAA acknowledgement, confidentiality form, latex allergy questionnaire, hand hygiene policy, fingernail policy, dress code policy, and regulatory compliance education.
- Unit-specific competency exams and medication/pharmacology competency testing.
- Color blindness testing within 12 months of start.
- References: Two professional references required, preferably supervisors within the last year.
- Employment application and skills checklist required.
Additional Notes
- Candidates must be able to float within the service line.
- No weekend requirement specified.
- No first-time travelers accepted.
- Must comply with all facility policies and complete required orientation and training modules.
- This is a travel/contract position with a critical access hospital rehab department.
