Description

Hospice RN - Contract Position

Location: Moab, Utah
Employment Type: Contract (13 weeks)
Shift: Days
Hours per Week: 32


Job Summary

Provide skilled nursing care and support to terminally ill patients and their families in accordance with individualized hospice care plans. Work as part of an interdisciplinary team to deliver compassionate, high-quality end-of-life care, primarily in patients' homes.


Key Responsibilities

  • Perform comprehensive patient assessments and coordinate individualized care plans.
  • Manage pain and symptoms consistent with hospice philosophy.
  • Educate and support patients and families on disease processes, self-care, and end-of-life issues.
  • Collaborate with physicians, social workers, chaplains, and volunteers.
  • Participate in on-call rotation (1-3 shifts per week, including 3 weekends during the contract).
  • Respond within 45 minutes to patient needs when on call.
  • Maintain accurate and timely documentation per state, federal, and organizational policies.

Required Qualifications

  • Valid Registered Nurse (RN) license in Utah
  • Minimum of 3 years hospice nursing experience, including experience within the last year
  • Current Basic Life Support (BLS) certification
  • Advanced Cardiovascular Life Support (ACLS) certification
  • Pediatric Advanced Life Support (PALS) certification
  • Neonatal Resuscitation Program (NRP) certification
  • Advanced Trauma Training (ATT) certification
  • Crisis Prevention Intervention (CPI) certification
  • Emergency Nursing Pediatric Course (ENPC) certification
  • Registered Nurse First Assistant (RNFA) certification
  • Trauma Nursing Core Course (TNCC) certification
  • Association of Women's Health, Obstetric and Neonatal Nurses (AWHONN) certification
  • National Institutes of Health Stroke Scale (NIHSS) certification
  • STABLE Program certification
  • Advanced Surgical Life Support (ASLS) certification
  • National Board of Surgical Technology and Surgical Assisting (NBSTSA) certification
  • Association of Pediatric Hematology/Oncology Nurses (APHON) certification
  • Certified Medical Assistant (CMA) certification
  • Certified Clinical Medical Assistant (CCMA) certification
  • Registered Medical Assistant (RMA) certification
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Preferred Qualifications

  • Experience with Hospicesoft or Careficient EMR systems.

Additional Information

  • Contract length: 13 weeks.
  • On-call shifts require availability to respond within 45 minutes.
  • Work week defined as Sunday 12:01 am through Saturday 11:59 pm.
  • Blue scrubs preferred.
  • Recognized holidays include New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
  • Level 4 Trauma facility.
