Registered Nurse - Telemetry

Location: Elmira, New York
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 07:00 AM - 07:30 PM

Job Description

• Med/Surg telemetry nurse on a 22-bed unit
• Patient mix includes Cardiac, Respiratory, Surgical, and Mental Health cases

Required Qualifications

• Valid RN license in New York
• Minimum 5 years of Telemetry nursing experience, including experience within the last year
• BLS certification issued by the American Heart Association
• ACLS certification
• Successful completion of the Centralus Core Competency Exam
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Onboarding & Compliance Requirements

• Completed skills checklist specific to the unit, updated annually
• Core Competency Exam for RNs with a minimum score of 80%
• COVID-19 vaccine proof or agency declination (optional)
• Immunizations and health requirements:

  • MMR: Positive IgG titer or 2 vaccine series required; must show results for each component
  • Varicella: Positive IgG titer or 2 vaccine series required
  • Hepatitis B: Positive antibody titer, 3 vaccine series, or specific declination form
  • Influenza vaccine or specific declination form (mask required if declined)
  • Tdap preferred within last 10 years or declination form
• TB testing: One of the following within last 12 months:

  • Two skin PPDs (one within 12 months, one within 3 months)
  • One Quantiferon/Spot test
  • For past positives, chest x-ray within 12 months may be required
• Fit test for approved respirator models within last 12 months (annual requirement)
• Physical/Health statement from an approved provider within last 12 months, including vitals and clearance with no restrictions
• Valid, unexpired US driver’s license or state ID

Background & Screening

• Agency background check within 30 days of start, including:

  • SSN address trace for all counties lived/worked in last 7 years
  • County and national criminal searches
  • FACIS 3 check
• 9+ panel drug screen within 30 days of start covering a broad range of substances
• 7 manual instant background checks required (specific to state and federal registries)

Documentation & Other Requirements

• Complete and accurate resume accounting for all employment and gaps for last 7 years
• Two professional references:

  • One supervisor reference from last 12 months
  • One supervisor or peer reference from last 3 years
• Signed and dated job description
• Verification of most recent employer (agency verification accepted)
• Color headshot photo (professional, no filters, JPEG format)
• Completion of various onboarding documents and acknowledgments including:

  • Health personal information form
  • Code of conduct
  • Confirmation of assignment form
  • Self-study packet and quiz
  • Sexual harassment acknowledgement
  • Parking registration form
• TB questionnaire and Epic questionnaire forms (specific to the facility)

Notes

• Extended travelers beyond one year must complete a health reassessment form
• Candidates with Canadian licenses must provide proof of valid TN visa or eligibility documentation

This position is ideal for an experienced telemetry RN comfortable with a diverse patient population and meeting comprehensive onboarding and compliance standards.
