Registered Nurse - Case Management (Travel)

Work Schedule

  • Monday to Friday, 8:00 AM to 4:30 PM
  • 40 hours per week

Job Description

  • Coordinate management of patient care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
  • Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
  • Enhance quality of patient management and satisfaction by promoting continuity of care and cost-effectiveness.
  • Integrate case management, utilization review, and discharge planning functions.
  • Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
  • Collaborate strongly with interdisciplinary teams (IDT).
  • Experience with MCG guidelines and discharge planning across acute care settings is required.

Required Qualifications

  • Valid RN license in Illinois
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum 2 years of Acute Care Case Management experience, including experience within the last year
  • Experience in a Long Term Acute Care (LTAC) setting
  • BLS certification through the American Heart Association
  • Experience with MCG guidelines
