Registered Nurse - Case Management (Travel)

- Monday to Friday, 8:00 AM to 5:00 PM
- 40 hours per week
Job Description
- Coordinate management of patient care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
- Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
- Enhance quality of patient management and satisfaction by promoting continuity of care and cost-effectiveness.
- Integrate case management, utilization review, and discharge planning functions.
- Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
- Collaborate strongly with interdisciplinary teams (IDT).
- Experience with MCG guidelines and discharge planning across acute care settings is required.
Required Qualifications
- Valid RN license in Indiana
- Minimum 5 years of recent Case Management experience, including experience within the last year
- Experience in Long Term Acute Care (LTAC) settings
- Acute Hospital Case Management experience
- Discharge Planning experience
- BLS certification issued by the American Heart Association (must be current and attached to application)
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
