Registered Nurse (Medical/Surgical/Telemetry) - New York, Elmira

Job Details

  • Specialty: RN Medical / Surgical / Telemetry
  • Shift: 7:00 PM – 7:00 PM (12-hour shift)
  • Contract Length: 13 weeks
  • Employment Type: Long-Term Assignment (LTA)
  • Work Environment: Primary and team nursing; ability to read basic rhythm strips required

Required Certifications & Training

  • BLS: American Heart Association (AHA) only; must be valid throughout assignment
  • ACLS: AHA only; must be valid throughout assignment
  • Stroke Training: 8 hours required, including NIH Stroke Scale (NIHSS) training (3-4 hours) plus additional stroke training hours
  • Fit Test: Must be completed within the last 12 months prior to start; annual requirement; PAPR provided if needed
  • COVID-19 Vaccine: Optional; proof of vaccination or agency declination accepted

Required Licenses & Credentials

  • State Nursing License: Verification completed within 30 days prior to start; must be valid for the position
  • Driver’s License or State ID: Valid, unexpired U.S. driver’s license or ID required
  • Professional Skills Checklist: Fully completed and updated annually for the assigned unit
  • Core Competency Exam: Must score 80% or higher on Core Competency I, II, and III for RNs

Required Health & Immunization Records

  • MMR: Positive IgG titer or two vaccine series; if negative/equivocal, vaccination series required with timelines for completion
  • Varicella: Positive IgG titer or two vaccine series; similar requirements as MMR for negative/equivocal results
  • Hepatitis B: Positive antibody titer, three vaccine series, or specific declination form; if negative/equivocal, declination form required
  • TB Test: One of the following within required timeframes:
    • Two skin PPDs (one within 12 months, one within 3 months), or
    • One Quantiferon/Spot test within 12 months
    • For past positive TB tests, chest X-ray within 12 months may be required
  • Tdap: Preferred but not required; if no record, specific declination form required
  • Physical/Health Statement: Within last 12 months; must include physician credentials, vitals, height, weight, Snellen eye chart, and statement of clearance without restrictions

Required Background & Screening

  • Agency Background Check: Must be run within 30 days of start; includes SSN address trace, county and national criminal checks, FACIS 3 check, and all aliases
  • Drug Screen: 9+ panel drug screen within 30 days of start, covering amphetamines, barbiturates, benzodiazepines, cocaine, opiates, oxycodone, phencyclidine, methadone, methaqualone, propoxyphene, and nitrites
  • 7 Instant Background Checks: Includes multiple federal and state exclusion and registry checks (manual upload required)

Required Documentation

  • Resume: Complete employment and education history for the last 7 years (or since graduation), with dates in MM/YY format
  • Professional References: Two references required:
    • One supervisor reference from employment within the last 12 months
    • One supervisor or peer reference from employment within the last 3 years
  • Employment Verification: Verification of most recent employer required; travel agency counts as one employer for consecutive assignments
  • Job Description: Signed and dated copy required
  • Color Headshot: Clear, straightforward photo with solid background; no filters or sunglasses; JPEG format
  • Code of Conduct, Personal Information Form, Confirmation of Assignment, Self-Study Packet, Sexual Harassment Acknowledgement, Parking Registration Form: Must be completed and submitted as part of onboarding
  • Epic Questionnaire and TB Questionnaire: Specific forms required (agency-specific)

Requirements / Qualifications

  • Valid RN license in New York
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum of 3 years of Medical/Surgical and Telemetry nursing experience
  • Medical/Surgical and Telemetry nursing experience within the last year
  • BLS certification issued by the American Heart Association
  • ACLS certification issued by the American Heart Association
  • 8 hours of stroke training, including NIH Stroke Scale (NIHSS) training
  • Demonstrated stroke nursing experience
