Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Average patient load: 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
- Must be willing to travel up to 75 miles from the home clinic, with an estimated weekly travel of 150-200 miles.
- Use of HomeCare HomeBase (HCHB) EMR required; proficiency within the last 6 months preferred. RNs must also be proficient in OASIS.
- Typical schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Some positions may have a 4x10 schedule (Friday-Monday).
- No guaranteed hours; floating to nearby clinics possible if patient census is low.
- Dress code: Scrubs.
Required Qualifications
- Valid Registered Nurse (RN) license in Massachusetts.
- Minimum of 1 year of hospice nursing experience within the last year.
- Experience in a Home Health setting.
- BLS certification issued by the American Heart Association or American Red Cross.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Proficiency with HomeCare HomeBase (HCHB) documentation system.
Schedule and On-Call Requirements
- No weekend rotation or on-call required for this assignment.
- Weekend and on-call requirements may vary by location and profession; discuss during interview if applicable.
Coverage Area
- Leominster, MA and a 30-mile radius.
Productivity Expectations
- Weekly units of productivity: 25-30.
Background and Compliance Requirements
- Must complete a standard background check covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches.
- Required state-specific clearances for Massachusetts, including MA Nurse Aide Registry Search and iCORI clearance.
- Fingerprinting required as per state mandates.
- Education verification required for nursing degree.
- Drug screening: 10-panel drug test required.
- Immunizations and health documents required:
- Physical exam within 12 months prior to start.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.
- Two professional references required.
- Verification of work history for at least the last 7 years or two most recent employers.
