Description

Physical Therapist Job Opportunity

Contract Length: 13 weeks
Shift: 8:30 AM – 5:00 PM
Hours per Week: 40

Required Qualifications

Valid Physical Therapist license in Pennsylvania
Allied License/Certification
Basic Life Support (BLS) certification

Required Screenings and Verifications

Background check
Drug screening
Educational and employment verification
License verification
Physical exam
Immunizations and health screenings including:
- Hepatitis B
- MMR (Measles, Mumps, Rubella)
- TdaP vaccine
- Varicella
- TB screening and annual TB questionnaire
- Latex health questionnaire
- Color vision screening
Competency exam and skills checklist
Core mandatory annual training
Motor vehicle report and valid driver’s license with car insurance
Various state and federal exclusion and sanction screenings (e.g., FACIS 3M, NJ Debarment, OFAC Sanction, SAM Sanction, PA Medicheck, PATCH, NJ Sex Offender Exclusion)
