Registered Nurse ICU - New York, Elmira

Job Details

  • Specialty: RN Critical Care
  • Unit: 20-bed ICU caring for cardiac, neurology, and respiratory patients
  • Shift: 7:00 PM – 7:00 AM (Night Shift)
  • Contract Length: 13 weeks (Long-Term Assignment)

Required Qualifications

  • Valid RN license in New York
  • Minimum 2 years of ICU (Intensive Care Unit) experience, with at least some ICU experience within the last year
  • BLS certification issued by the American Heart Association
  • ACLS certification
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Health & Safety Requirements

  • COVID-19 Vaccine: Optional; proof of vaccination or agency declination accepted
  • MMR: Positive IgG titer or 2 vaccine series required; titers must show results for measles, mumps, and rubella individually
  • Varicella: Positive IgG titer or 2 vaccine series required
  • Hepatitis B: Positive antibody titer, 3 vaccine series, or facility-specific declination form required
  • TB Testing: Must have one of the following within required timeframes:
    • Two PPD skin tests (one within 12 months, one within 3 months) OR
    • One Quantiferon/Spot test within 12 months
    • For past positive TB tests, chest X-ray within 12 months may be required
  • Tdap: Preferred but not required; if no record, facility-specific declination form needed
  • Fit Test: Must be completed prior to start and within the last 12 months; facility provides specific approved mask types and on-site fit testing if needed
  • Physical/Health Statement: Within last 12 months; must include physician credentials, vitals, height, weight, Snellen eye chart, and statement of clearance without restrictions
  • Drug Screen: 9+ panel drug screen within 30 days of start, covering a comprehensive list of substances

Documentation & Onboarding Requirements

  • State License Verification: Required for submission
  • Resume: Must include all employment and gaps for last 7 years, education history with degree titles and graduation dates, and locations for all assignments
  • Professional References: Two required—one supervisor reference from last 12 months and one supervisor or peer reference from last 3 years
  • Background Check: Must be run within 30 days of start, including SSN address trace, county and national criminal checks, and FACIS 3 check
  • Core Competency Exam: Must score 80% or higher on nursing core competency exams
  • Skills Checklist: Completed for the ICU unit and updated annually
  • Color Headshot: Recent, professional photo without filters or sunglasses
  • Employment Verification: Verification of most recent employer required (agency counts as one employer)
  • Job Description: Signed and dated copy required
  • Health & Safety Forms: Includes TB questionnaire, drug/tobacco acknowledgment, and other facility-specific forms found in document library
  • Additional Facility Forms: Code of conduct, personal information form, sexual harassment acknowledgment, parking registration, confirmation of assignment, self-study packet and quiz
