Registered Nurse - Case Management (Travel)

Job Description
- Collaborate with interdisciplinary and healthcare team members to facilitate patient care.
- Lead safe discharge planning to achieve clinical and financial outcomes.
- Coordinate care and services throughout the acute care episode and across the continuum.
- Must be willing to float to other units and hospitals.
- Work schedule: 36 or 48 hours per week, including holidays as required and every other weekend.
- Patient-facing role.
- On-site position (not remote).
Required Qualifications
- Valid Registered Nurse (RN) license in Alabama.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing.
- Minimum 2 years of case management experience, including experience within the last year.
- Experience in utilization management.
- Current Basic Life Support (BLS) certification issued by the American Heart Association or Red Cross.

- Clinical role.
- Electronic Medical Record system: Meditech.
- Dress code: Solid navy scrubs.
