Description
CT Technologist Position - Iowa, Davenport
Job Details
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 13 weeks
- Shift: 11:00 PM – 7:30 AM
- Weekend Requirement: Every other weekend shift
Required Qualifications
- Valid CT Technologist license in Iowa
- Minimum of 1 year of CT Technologist experience within the last year
- ARRT certification
- BLS certification issued by the American Heart Association
Health and Safety Requirements
- Fit test
- Trinity 10 Panel Drug Screen (10-panel, no THC; completed within 30 days of start)
- COVID-19 vaccine proof (including booster by Sept 2025) or approved exemption/declination (declination not accepted in CA or IL)
- MMR: Proof of 2 vaccinations or IgG titer lab report
- Hepatitis B: Must have started vaccination series prior to start (titers not accepted)
- Influenza vaccine (optional)
- 2 Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
- Varicella: Proof of vaccination or IgG titer lab report
- Physical/Health Statement confirming fitness to work (exam within last year)
- Tdap vaccine or declination if no pediatric interaction
Background and Compliance
- Federal, nationwide, and county criminal background check (last 7 years), including SSN verification, dated within 30 days of start
- Education verification of highest academic degree or diploma/GED
- EPLS (Excluded Parties List System) check dated within 30 days
- OIG (Office of Inspector General) exclusion check dated within 30 days
- NSO (National Sex Offender) check dated within 30 days; includes statewide check for Michigan if applicable
- Conflict of Interest Disclosure form
- Code of Conduct form
- Confidentiality and Information System Security Agreement
- Integrity and Audit Services form
- Non-Employee Upload form (pages 1-3 and 28)
- System Access form (to be completed and uploaded one week before start)
- Child and Adult Dependent Abuse Training (Iowa only)
- Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries) within 30 days prior to start
- OFAC (Office of Foreign Assets Control) sanctions check within 30 days

- Two professional references
- Specialty exam results
- All required scanned documents and verifications as listed above must be uploaded to the credentialing system prior to onboarding
