Description
RN-Care Manager
Location: Chapel Hill, North Carolina
Employment Type: Clinical Nursing
Shift: Days
Hours: 12 hours/day, 36 hours/week
Contract Length: 13 weeks
Job Summary
Provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs. Enhance quality of patient management and satisfaction, promote continuity of care, and support cost-effectiveness through case management, utilization review, and discharge planning.
Key Responsibilities
- Prioritize patients and identify new admissions daily.
- Conduct and document patient assessments and care plans according to departmental guidelines.
- Participate in daily Care Management Touchpoints and CAPP meetings to discuss patient progression and discharge planning.
- Communicate with care team members, including social workers and utilization managers, to address discharge readiness and barriers.
- Attend weekly Complex Care Meetings to present complex cases, collaborate on problem-solving, and monitor follow-up actions.
- Coordinate family meetings and provide education on community resources and post-acute care options.
- Monitor patient progression, assess discharge readiness, and ensure appropriate post-discharge planning and authorizations.
- Refer administrative tasks to Care Management Assistants as appropriate.
Required Qualifications
- Valid Registered Nurse (RN) license in North Carolina.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN) from an accredited school of nursing.
- Minimum 1 year of case management experience, including case/care manager and discharge planner roles within an acute hospital setting.
- Experience in acute care hospital case management and discharge planning.
