PT Home Health - Oregon (Travel/Contract)

Job Details

  • Location: Seaside, Oregon (and surrounding areas)
  • Specialty: Therapy/Rehabilitation (Physical Therapy)
  • Employment Type: Travel/Contract
  • Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
  • Contract Length: 13 weeks
  • Patient Load: 1:1 ratio; may visit up to 6 patients per day; 30 productivity points standard
  • Start Date: ASAP
  • Weekend Requirement: No
  • On-Call: No
  • Floating: No floating between units required
  • Block Schedule: Not accommodated
  • Locals Accepted: Yes, within 50-75 mile radius
  • Pending License Accepted: Yes
  • Guaranteed Hours: Facility may call off up to one shift every two weeks
  • Transportation: Traveler must have own transportation; daily travel radius up to 60 miles (60-90 minutes total drive time)
  • COVID Vaccination: Primary series plus one booster required

Required Qualifications

  • Valid Physical Therapist license in Oregon (pending licenses accepted)
  • Minimum 1 year of Physical Therapist experience, including experience within the last year
  • Experience in a Home Health setting
  • BLS (Basic Life Support) certification required
  • Must have own transportation

  • Type of Facility: Home Health
  • Attire: Business casual; scrubs allowed but not required; no jeans
  • Charting System: EPIC Remote Client (laptop) and Rover (phone app)
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Health & Safety Requirements

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
  • Fit mask testing required annually (completed onsite if no recent record)
  • Immunizations and documentation required:
    • COVID vaccination (primary series + booster)
    • Flu vaccine during flu season (declinations accepted)
    • Tetanus/Diphtheria/Tdap within 10 years (declination accepted)
    • Negative TB test or chest X-ray within 12 months (annual updates required)
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer
  • Drug screening: 9-panel drug screen within 12 months of start (instant acceptable)
  • Respiratory safety clearance required annually for respirator use

Background & Compliance

  • Background check within 90 days covering all locations lived/worked in past 7 years; must include SSN trace, death master index, sex offender registry, education verification, motor vehicle record, and Oregon Abuse/Neglect search if applicable
  • Education verification required
  • Employment application covering at least last 5 years of employment
  • Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting
  • State-specific searches and attestations as required by Oregon and Washington laws (if applicable)

Additional Notes

  • Facility cannot accommodate alternative shift patterns such as 4x10 or 3x12
  • No weekend or on-call shifts required
  • Traveler may be called off for up to one shift per pay period
  • System groups clients to minimize drive time
  • Annual regulatory and specialty-specific training modules may be required for other settings but not required for this home health position
  • Badge photo required (passport style with neutral background)
  • Must comply with float policy acknowledgment (may be required to float to other nearby facilities, but not expected for this role)
